Deadline August 31, 2004. Effective date October 1, 2004.
Resets unmet deductibles January 1, 2005.
Must notify bookkeeper of Family Status Change within 30 days of event.

Vision Insurance: Spectra, Inc./Vision. Open Enrollment will be in November 1, 2004 for an
effective date of January 1, 2005. You need to call 490-6230 for information.

Dental Insurance: The deadline to sign up or add to your dental insurance is Dec. 17, 2004, for an effective date of Feb. 1, 2005. This is the only open enrollment period of the year.

Cafeteria Plan: Our Cafeteria Plan year starts June 1, 2004 through May 31, 2005. Enrollment for new employees will be in Aug. or early Sept. by USAble Life representatives.

Automobile and Homeowners Insurance: Enrollment is open anytime throughout the year. To enroll or
