*Proof of address must be a current (last 30 days) gas, water, electric bill,

Base Housing verification form, home purchase closing papers or lease.  Homeless does not require proof of residence.

All New Students Grades Pre-Kindergarten thru Twelve are

Required to Register at their Assigned School.

        May accept registrar statement, baptismal            certificate, passport, affidavit, Military                       ID, or previous  school records in lieu of              Birth Certificate

ü Current Proof of Address*

ü Legal Documents (if applicable)

ü Social Security Number

ü Transcript or Last Report Card

Returning students who have

changed addresses but still reside

in the attendance zone must
